My mom was a patient here for about a month and a half. I was so thrilled with the results we saw from her rehabilitation here! The staff here is so wonderful and was very patient with my mom. I want to shout out a few individuals that really warmed my heart: Nautica, Yamii, Celess, Amandeep, Taylor, and Wendy! Thank you all for your dedication to helping my mother reach her goals. From the bottom of our hearts, we are grateful.

During my 3 week rehabilitation stay at Willow Creek Health Center, I had the pleasure of meeting some very wonderful people. Dr Z is a wonderful caring doctor, my physical therapy team were so caring and helpful. Thank you Martin, Cindy, and I’ve seamed to have forgotten the other lady’s name. My nurses were very hardworking and caring people. Thank you, Rose, Cindy, Karen, and Christopher, Christopher is a nurses assistant. There were other people also that were on the medicine crew. I am doing great and still following your advice. Thank you all from the bottom of my heart for you caring and professionalism. Sincerely Jeffery G

Not happy how they had us bring dad two days early.As soon as I got him home I noticed that his nose was running. I gave him a Covid test and he was positive. Now three people from our family are positive for Covid. They should have tested him before releasing him

Residents seem well taken care of, helpful staff. Facility is well kept up.

My first time experiencing this facility, it’s very well kept cleanliness, staff are very helpful as to looking for residents. Even talking with the residents..they claim to have always be on time for their appointments.

It’s very nice inside but the room my uncle is in has holes in the walls and looks very old and not together.

Amazing facility.

If I could give negative stars they would get -5 I have a loved one that I went to go visit yesterday. They had been sitting in a soil diaper for an hour and a half. The facility supposedly was out of extra large so I went to the store to get her some after about 20 minutes of getting back, they finally changed into the ones that I bought from the store. I later get a call from the Director of nursing stating it was an error. It was a mistake. The CNA’s knew she wasn’t trained correctly and all is good. OK well if that’s the case, how come when I came back 30 to 45 minutes later, she was still in a soil diaper, and you put her in the diapers that I brought how come she wasn’t fresh clean and in an extra large diaper from your facility, doesn’t add up this is just ONE incident I happen to walk in on almost 2 hours in a soiled diaper

Spent several months attending this facility. Staff is friendly and has a passion for caregiving. The facility is well kept and very clean. Unlike some facilities, this one functions and operates as one whole unit. Supportive and cooperative with all the residents. Would recommend someone looking for a facility in Clovis

Without question, Willow Creek Healthcare Center is the finest rehab facility in the San Joaquin Valley. I was at a previous five star facility that failed me miserably and landed me back in the hospital. When I was sent To Willow Creek I was apprehensive. Soon I realized how lucky I was, to have been sent To Willow Creek. The level of care from the nurses to the CNA’s to the physical therapist is without questioning the best Care I’ve ever received. The staff is kind and caring and goes above and beyond to help you achieve your goals. Willow Creek is in an extraordinary facility with an Incredible loving staff . They will give you the best opportunity to achieve your goals. I highly recommend Willow Creek to anyone in need of Care.

The staff are amazing, maintenance guy is super nice and way helpful and the manager guy Sean he is super friendly and helpful also on point and knows so much!!

Willow Creek Facility is a nice and comfortable place for elderly people who need care. The staff truly care for the people they serve and look after. I highly recommend this facility to anyone looking for a place that can provide the care for their family members need.

Im really disappointed this time with Willow Creek. My mom has been there several times, dont know if ownership change or new service. First of all I noticed my mom her room very dirty, I dont know when they clean but same wrappers on floor for 2 days. The other is she was never accommodated with water daily or on her table. I always had to ask and they said they would go get it and never did. The worst part was the Social Service Director Jennifer Soriano. When I wanted my mom released she was rude and rolling her eyes. She also said she didn’t know when she can get my mom released because she had other things to do meetings, etc and so much paperwork in which she never filled out the RN had to do it. This girl needs to go to a PR class or something to get a smile in her face instead of eye rolling with her fake lashes. Last when I got her released no one helped to transport to car or even get her ready I had to do it. I suggest you re train your staff. Im sure you already lost alot of patients and I would not recommend this healthcare. Like I said I used before but never have we been treated so poorly.

Administration (social services)worst I’ve ever seen. Very rude. Place was very dirty.

So far my mom is extremely happy with this location. We have been looking for help since Dad passed so I can keep working. Kingston Bay neglected doctor orders and it created a massive bed upset, past stage 4!! Willow creek has been very helpful in getting it to heal up

My father was there recently before going back to Kaiser for a second admission, as Williw Creek was holding up on sending him for proper emergency care. The evening nurse was rude and unprofessional in his care and did not want to handle his care or him as a patient. To top it off, as he was being admitted again at Kaiser, they gathered up all his belongings and threw them in their public dining hall. My dad’s personal belongings were gone through, and stuff was stolen. I was given Joanne’s number in social services to report the items stolen, and she said that unless I had the receipts, his stuff can’t be reimbursed. Also, his personal information from Willow Creek and Kaiser, with all his patient identifiers, including POLST, was left exposed for anyone to see. That’s a HIPAA breach, and I will contact The Joint Commission, the state, and the Better Business Bureau regarding the lack of professionalism at this establishment.

My father was there got sick and was sent out to Kaiser when to pick up his stuff was all gone. They threw it in the dinner room. Called the facility up and they asked us if we had proof of purchase for all his stuff that was taken. They couldn’t put it in a box!!
